Leadership and administration to advance translational science: Environmental scan of adaptive capacity and preparedness of Clinical and Translational Science Award Program hubs

COVID-19 reinforced the need for effective leadership and administration within Clinical and Translational Science Award (CTSA) program hubs in response to a public health crisis. The speed, scale, and persistent evolution of the pandemic forced CTSA hubs to act quickly and remain nimble. The switch to virtual environments paired with supporting program operations, while ensuring the safety and well-being of their team, highlight the critical support role provided by leadership and administration. The pandemic also illustrated the value of emergency planning in supporting organizations’ ability to quickly pivot and adapt. Lessons learned from the pandemic and from other cases of adaptive capacity and preparedness can aid program hubs in promoting and sustaining the overall capabilities of their organizations to prepare for future events.

Background
Countries worldwide are experiencing a third wave of the coronavirus disease 2019 (COVID-19) pandemic. Government-imposed restrictive measures continue with undetermined effects on physical and mental health.
AimsTo compare child and adolescent mental health services (CAMHS) referrals over 11 months (January–November) in 2020, 2019 and 2018 and examine any impact the different phases of the COVID-19 restrictions might have on referral rates.
MethodMonthly CAMHS Health Service Executive data were examined, covering a catchment population of 260 560 or 12.7% of all youth (age group 0–18 years) in Ireland. The total number of urgent and routine referrals, appointments offered, rates of non-attendances and discharge outcome are presented.
ResultsThere was a significant drop in referrals in 2020, compared with prior years (χ2 = 10.3, d.f. = 2, P = 0.006). Referrals in 2020 dropped from March to May by 11% and from June to August by 10.3%. From September, both routine and urgent referrals increased by 50% compared with previous years (2018/2019), with the highest increase in November 2020 (180%). Clinic activity also increased from September, with double the number of out-patient appointments offered, compared with previous years (χ2 = 5171.72, d.f. = 3, P < 0.001) and lower (6.6%) rates of non-attendance (χ2 = 868.35, d.f. = 3, P < 0.001).
ConclusionsIn 2020, following an initial decline, referrals to CAMHS increased consistently from September. Such unprecedented increase in referrals places further strain on services that are already underresourced and underfunded, with the likelihood of increased waiting lists post COVID-19. It is envisaged that once the pandemic is over, resources will be even more constrained, and CAMHS will be urgently in need of additional ring-fenced funding.

ABSTRACT IMPACT: This work will inform the ongoing development of adaptive capacity and preparedness of the CTSA Program and other clinical and translational research organizations in their quest of improving processes that drive outcomes and impacts, shaping effective programs and services, and strengthening their emergency readiness and sustainability. OBJECTIVES/GOALS: -Share the progress and preliminary findings of an ‘Adaptive Capacity and Preparedness of CTSA Hubs’ CTSA Working Group; -Improve our awareness and understanding of the efficient and effective changes helping CTSA hubs build robust capacity to address METHODS/STUDY POPULATION: A multi-case study including: - Triangulating multiple sources of information and mixed methods (survey/interviews of research administrators, researchers, evaluators, and other key stakeholders), literature review, document and M&E system information analysis, and expert review; - Describing CTSA hubs’ experiences as related to research implementation, translation, and support during the time of emergency; - Administering a comprehensive survey of the CTSAs addressing their challenges, lessons learned, and practices that work in various program components/areas. Data collection includes aggregate and cross-sectional data, with representation based on CTSA size, maturity, and population density. RESULTS/ANTICIPATED RESULTS: The described approach shows sound promise to investigate and share strategies and best practices for building adaptive capacity and preparedness of CTSAs -- across various scientific sectors, translational research spectrum, and the goals outlined by NCATS for the CTSA program. The anticipated results of this research will include the identified/shared innovative solutions and lessons learned for this rapidly emerging, high-priority clinical and translational science issue. ‘High-quality lessons learned’ are those that represent principles extrapolated from multiple sources and triangulated to increase transferability to new contexts and situations. DISCUSSION/SIGNIFICANCE OF FINDINGS: The project provides useful knowledge and tools to research organizations and stakeholders across multiple disciplines -- for mitigating the impact of the COVID-19 disaster via effective adjusting programs, practices, and processes, and building capacity for future successful, ‘emergency ready and responsive’ research and training.

The scarcity of Romano-British human remains from north-west England has hindered understanding of burial practice in this region. Here, we report on the excavation of human and non-human animal remains1 and material culture from Dog Hole Cave, Haverbrack. Foetal and neonatal infants had been interred alongside a horse burial and puppies, lambs, calves and piglets in the very latest Iron Age to early Romano-British period, while the mid- to late Roman period is characterised by burials of older individuals with copper-alloy jewellery and beads. This material culture is more characteristic of urban sites, while isotope analysis indicates that the later individuals were largely from the local area. We discuss these results in terms of burial ritual in Cumbria and rural acculturation. Leveling the Odds: The Effect of Quality Legal Representation in Cases of Asymmetrical Capability

How much does attorney quality influence the outcome of cases in which one litigant is significantly more capable than the other? Using a unique dataset of all asylum merits decision from 1990 to 2010, we find that high quality representation evens the odds for asylum applicants and that not being represented by legal counsel is actually better than being represented by a poor lawyer. In this analysis, we draw on a modified party capability theory and create new measures of attorney capability. We find that variation in attorney capability is a primary driver of the disparity in asylum outcomes in U.S. immigration courts and that a likely causal mechanism for this influence is the judge-specific reputation of an attorney.

Globally, areas categorically known to be free of human visitation are rare, but still exist in Antarctica. Such areas may be among the most pristine locations remaining on Earth and, therefore, be valuable as baselines for future comparisons with localities impacted by human activities, and as sites preserved for scientific research using increasingly sophisticated future technologies. Nevertheless, unvisited areas are becoming increasingly rare as the human footprint expands in Antarctica. Therefore, an understanding of historical and contemporary levels of visitation at locations across Antarctica is essential to a) estimate likely cumulative environmental impact, b) identify regions that may have been impacted by non-native species introductions, and c) inform the future designation of protected areas under the Antarctic Treaty System. Currently, records of Antarctic tourist visits exist, but little detailed information is readily available on the spatial and temporal distribution of national governmental programme activities in Antarctica. Here we describe methods to fulfil this need. Using information within field reports and archive and science databases pertaining to the activities of the United Kingdom as an illustration, we describe the history and trends in its operational footprint in the Antarctic Peninsula since c. 1944. Based on this illustration, we suggest that these methodologies could be applied productively more generally.

Historically, U.S. asylum policy has reflected both an effort to provide safe haven for deserving asylum seekers and the intent to promote national security and domestic policy priorities. A growing body of empirical evidence suggests that asylum outcomes, at least in the aggregate, have been weighted more heavily by foreign policy considerations than humanitarian concerns. Since the mid 1990s, the United States government has reformed the asylum system in response to concerns of abuse by economic migrants, burgeoning caseloads, and national security threats. Although, as Davergne (2008) points out, “the most reviled of asylum seekers of the global era is the ‘economic refugee,’ under suspicion of fleeing poverty and poor prospects in search of a better life” rather than fleeing because of the fear of persecution (65), in the wake of terrorist attacks on the United States in 1993 and 2001, the fears of economic opportunists abusing the system have combined with the broader fear of potential terrorists gaining legal entry into the country through an overburdened asylum system. Since 1995, Congress has passed two major acts to reform the asylum process in reaction to these fears. Both the 1996 Illegal Immigration Reform and Immigrant Responsibility Act and the 2005 Real ID Act were passed to prevent economic migrants and individuals who may pose security risks from entering the country on false claims of asylum. Following September 11, the U.S. government has pursued prosecution for documents fraud among asylum applicants and aggressively enforced safe third country requirements. And, like our European counterparts, the United States has increasingly taken more deterrent and preventative actions to discourage asylum applicants from choosing it as a target for asylum and prevent potential applicants from reaching its ports of entry. Critics fear that the draconian measures adopted in response to these fears are overly broad and worry that worthy applicants have been turned away at the border or denied asylum with increasing frequency, thus leading them to face the real possibility of torture and other forms of persecution. In this article, we examine changes in U.S. asylum policy and whether the heightened security concerns after September 11 have significantly influenced the U.S. asylum process and outcomes in U.S. immigration courts.

Summary
Introduction
Traumatic brain injury (TBI) is one of the leading causes of morbidity and mortality in youth worldwide. Though much remains to be learned, research focused on pediatric TBI has flourished in the last two decades. The current chapter will emphasize recent scientific literature relevant to understanding the consequences of moderate to severe TBI and the role of neuropsychological assessment in particular in characterizing and managing these difficulties. Epidemiological, pathophysiological, and intervention data will be highlighted as well. Outcomes and clinical care after uncomplicated mild TBI in children can be expected to differ from more severe injury; comprehensive reviews of this literature are available elsewhere [1, 2].
Epidemiology
In the USA, more than one million children and adolescents sustain TBI each year. Among children aged 0 to 14 years, TBI accounts for approximately 2700 annual deaths, 37,000 hospitalizations, and 435,000 emergency department visits [3]. Not surprisingly given such frequency, the associated financial costs are considerable. Annual hospitalization charges alone exceed $1 billion [4]. Rates and costs of all childhood TBI are undoubtedly much larger, as many milder injuries go unreported entirely or are treated in outpatient settings and remain unaccounted for in hospital-based estimates.
The incidence of TBI varies by severity, with mild TBI comprising 80 to 90% of all treated cases. Throughout childhood, boys are at considerably greater risk for TBI than girls.
What's in a name: Crinipellis, the final resting place for the frosty pod rot pathogen of cocoa?

Confusion still surrounds the identify and even the common name of the causal agent of frosty pod rot of cocoa, nearly 70 years after its placement in the genus Monilia, and over 20 years since its transfer to the new genus Moniliophthora, which was erected to accommodate what was considered to be a basidiomycete anamorph. Based on molecular and cytological evidence, we confirm that the fungus belongs in the Basidiomycotina and is closely related to the witches' broom pathogen of cocoa, Crinipellis perniciosa (Tricholomataceae: Agaricales). However, the only known reproductive stage in the life-cycle does not, in fact, represent the anamorph since meiosis occurs during sporogenesis and germination. The meiospore is interpreted to be a radically modified basidium, which performs a sexual, dispersal and resting function. The genus Moniliophthora, therefore, can no longer be maintained and must be reduced to synonymy with Crinipellis. The new combination, Crinipellis roreri, is proposed.

Size-monodisperse, stable 15 Å diameter silicon nanocrystals were synthesized in significant quantities using supercritical octanol as a capping ligand. The silicon nanocrystals exhibit an indirect band gap with discrete electronic transitions in the absorbance and photoluminescence excitation (PLE) spectra. The octanol-capped clusters show efficient blue band-edge photoemission with a luminescence quantum yield of 23 % at room temperature.
